**Exclusions:** - Drug toxicity associated with harm in surgical or medical care, not elsewhere classified - Injury or harm arising following infusion, transfusion or therapeutic injection, not elsewhere classified - Reactions or intoxications due to drugs administered to fetus or newborn - Opioid intoxication - hypersensitivity reaction to correctly administered drug - Alcohol intoxication - pathological drug intoxication
