Personal history of maltreatment

**Definition:** Personal history of non-accidental acts of physical force, forced or coerced sexual acts, verbal or symbolic acts, or significant caregiving omissions that result in harm or have a reasonable potential for harm. These categories are applied to the victim of the maltreatment, not the perpetrator.

**Exclusions:** - History of spouse or partner violence
