Clinical Global Impression Original Version Questionnaire

**Definition:** A standardized survey developed by W. Guy in 1976 which is used to evaluate the efficacy of a therapeutic intervention. This instrument consists of 3 sections pertaining to the severity of the patient's illness, their level of improvement and a comparison of the therapeutic effect versus the side effects. The questionnaire is completed by a clinician familiar with the patient's medical history.
