**Definition:** A standardized survey developed and copyrighted by Kelly Posner and colleagues with The Research Foundation for Mental Hygiene in 2008 which is used to evaluate a patient's suicidal tendencies. The first three sections pertain to the patient's suicidal ideations and behaviors while the fourth section assess any actual suicide attempts.
