**Definition:** A standardized, multipurpose, general health survey developed by John E. Ware and copyrighted by Medical Outcomes Trust and Quality Metric Inc. in 1988 and 2002 which is used to assess the patient's functional health and overall well-being. This instrument contains 36 questions to be completed by the patient. For each question the patient must choose from five different responses.
