**Definition:** An aggressive, castration-resistant, histologically distinct, metastatic carcinoma arising from the prostate gland. It is characterized by the presence of a pure population of cytologically bland malignant epithelial cells with moderate to abundant cytoplasm and rare mitotic figures. The prognosis is poor.
