**Definition:** An intra-acinar and/or intraductal neoplastic epithelial proliferation in the prostate gland that has some features of high-grade prostatic intraepithelial neoplasia but exhibits much greater architectural and/or cytological atypia, typically associated with high-grade, high-stage prostate carcinoma.
