**Definition:** Involvement of lymph node regions on both sides (above and below) of the diaphragm. This may be accompanied by extra-lymphatic extension in association with adjacent lymph node involvement (i.e., local extension from a lymph node area into a nearby organ) and/or involvement of spleen. The medical record states there are constitutional symptoms. This definition applies to both tiers (tier 1 and 2).
