**Definition:** A standardized questionnaire, developed in 1994 at a Consensus Conference of the Working Group on Sepsis-Related Problems of the European Society of Intensive Care Medicine in Versailles, published in 1996, and renamed to the Sequential Organ Failure Assessment questionnaire, that assesses and scores the function of several body organ systems in patients with sepsis and critically ill patients without sepsis.
