**Definition:** A finding on a plain radiograph that shows an upturned cardiac apex due to right ventricular hypertrophy and a concave pulmonary arterial segment due to reduced pulmonary artery outflow from subpulmonic stenosis. The cardiac remodeling takes the appearance of a boot. It may be a characteristic finding in tetralogy of Fallot.
