PC.02 - Planning Care

PC.02 requires hospitals to develop a comprehensive plan of care for each patient based on a complete assessment, incorporating the patient's needs, goals and preferences. The plan must involve an interdisciplinary team, be documented clearly, and be updated as the patient's condition changes to ensure safe, coordinated delivery of services.
