**Definition:** A WHO grade 3 pleomorphic xanthoastrocytoma characterized by the presence of five or more mitoses per 10 high-power fields. Necrosis may be present. Patients have shorter survival rates when compared to those with WHO grade 2 pleomorphic xanthoastrocytoma.
